The Addictions Coach: Why Do We Buy More Than We Need? A look at Compulsive Shopping.

Many addictions are easy to spot because they are addictions to obvious destructive substances or behavior. A shopping addiction can be harder to recognize.

“Even though shopping is legal and, in fact, encouraged in many ways, there’s a lot of shame with admitting that you have a problem with it,” said Terrence Shulman, author of “Bought Out and Spent.” “People are embarrassed that they are shopping themselves into debt or are out of control, or they fear they’ll be judged for being materialistic.”

Shulman, who runs the Shulman Center for Compulsive Theft, Spending and Hoarding in Michigan feels that it’s a growing problem. The center offers help on a national level.

“I think, increasingly, both individually and collectively, something is going on,” he said. “People are becoming more in debt and hoarding more.”

Oscar Sida, instructor and Human Services Program coordinator at UNLV, said that, typically, shopping addictions fall into the category of “process addictions,” any kind of behavior that is pathological but doesn’t have a specific mental illness associated with it. Very often, it is used as a coping mechanism.

“It’s pretty simple,” Sida said. “When we buy things, it makes us feel better.”

Heather Marianna, who was featured in the Oxygen channel reality series “My Shopping Addiction,” used shopping to fill a void when she moved to Las Vegas.

“When I moved here, I didn’t have any friends, and I was just bored,” Marianna said. “I went and shopped at Crystals every day because I didn’t have anything else to do.”

The purchases weren’t destroying her financially, as she had recently received a large inheritance, but she said it was still problematic.

“When we’re looking to determine if the shopping is an addiction, we look for the things that are common with any addiction,” Shulman said. “Is it an ongoing problem? Is it secretive? Is it causing negative consequences? Have there been unsuccessful attempts to stop? If it looks like a duck, walks like a duck, and quacks like a duck, it might be a duck.”

Psychologists recognize several categories of shopping addicts. There are compulsive shoppers who shop to distract themselves from their feelings and codependent shoppers who shop to gain love and approval. There are also bulimic shoppers who repeat a cycle of buying and returning items. There are trophy shoppers who are seeking the perfect accessory for outfits or some other luxury item. Similarly, there are image shoppers who buy expensive cars, watches or other highly visible items or who will pick up a tab to reinforce their status. On the other end of the scale are bargain shoppers who buy things they don’t need just because they find a good deal.

One doesn’t need to be rich to have a shopping addiction. Sida notes that among the many things that might make a person more vulnerable to addiction are genetics, past trauma and even poverty.

“If you’re poor, you have less access to services,” Sida said. “There’s more psychological stress than normal and for a longer time.”

It’s also possible to have a physical injury trigger a shopping addiction.

“Maybe you were an active person who suddenly has limited mobility,” Sida said. “You might end up shopping too much online because you’re using it to fill the space that used to be used for physical activities.”

Sida said that there is no one-stop treatment for the issue, which may have multiple causes and is very likely to be paired with other issues.

“You should develop an individualized treatment,” Sida said. “That might mean having more than one health professional helping you out.”

For Marianna, the issue has been mostly resolved, although she still does quite a bit of shopping.

“I’ve started a beauty company, Beauty Kitchen,” the Boulder City resident said. “I’m still obsessed with buying, but now it’s for more materials to make more products.”

This is a first hand, personal account of an opiate user’s detox from Oxy, then Heroin and then Methadone. The most trying detox of the three may surprise you.

In 2009 John moved to Miami having never touched an opiate in his life, but after living in the pain pill capitol of Florida for over a year his curiosity got the best of him. A coworker turned him on to 15mg Roxy which rapidly turned into a habit. For the first 90 days John didn’t seem to detox when he didn’t have the pills, just a little tired. That’s when the whole game changed. Getting bored with the 15mg Roxies, John raised the bar to 30mg Roxy and within weeks increased to two 30mgs at a time.

Around mid 2010, Miami and all of South Florida really cracked down on local pill mills which led to John’s detox from Roxy. The scientific side of this is Roxy has a shorter half-life than other opiates so it leaves the body quicker so you get sick faster and it is very intense, but it is also over much quicker. With the Roxy, just getting through the first 72 hours is the key. Being a touring musician John left Miami for 5 weeks and seemed to have dodged a major bullet. So you would think.

Within 2 weeks of returning from the road, John started looking for pills again with no luck. That’s when his old dealer turned him on to Heroin which was cheaper and much more available. In John’s mind he had struck gold, a $100 a day roxy habit was now a $40 a day heroin habit. But true to form, that $40 a day heroin habit quickly turned into a $80 a day habit and then right back to $100 a day.

This usage lasted 2 years until personal and financial issues put the brakes on his party. This is where our first detox comparison comes in. John had quit Roxy cold turkey and suffered for 48 hours so he went into this situation with the same mindset. Huge mistake! At about the 24 hour mark from his last heroin dose the detox grabbed him physically and began 7 days of living  Hell. The first 48 hours of this new heroin detox immobilized John. He couldn’t get out of bed while violently throwing up and couldn’t even keep liquid down which contributed to major dehydration. Days 3 and 4 weren’t any better. Still not being able to eat, John had no energy at all. It took every ounce of will just to walk to the kitchen and bathroom. Thinking he was going to die, John suffered through the insomnia and night sweats until Day 6. Keep in mind, this detox has been much longer than the Roxy detox. Day 6 detoxing from Heroin seemed to give John some hope. Still fatigued with zero energy, John’s other physical symptoms had gone. Now it was just dealing with being tired. By Day 9 he was fine. But how soon we forget the bad part of addiction.

After 90 days of no Heroin or opiates, John found himself craving a few bags of Heroin and temptation gave in and within days he was in full blown usage again. Over the past year he knew he had to stop. He tried a few times but all the memories of the prior detox reminded him that help was needed. He could not go through that 7 day period again. After researching his options John decided to give Methadone a try and at first it worked like magic. 24 hours after his last Heroin dose John took his first dose of Methadone. Expecting the worst, John entered Day 1 with tons of anxiety but no sickness occurred. He continued the Methadone daily and there was never any sickness. Just a little lethargy, but no sickness. After 2 weeks on Methadone John felt safe to taper down and stop.

This is where our final detox comparison comes in. Keep in mind that John only used Methadone for 2 weeks. Methadone has a much longer half-life and can last upwards of 36 hours before it even begins to leave the body. This can give a false sense of security when stopping. You will detox from Methadone and it could take 3 days before you began to feel sick. This is exactly what happened to John. Day 1 and 2 after stopping Methadone showed no signs of detox. But Day 3 was a different story. Complete fatigue set in and John began to fear the worst. Day 4 and 5 was another 48 hour window of complete fatigue. By Day 6 the stomach issues were back in full force along with the night sweats. Day 7 welcomed back the insomnia and for the next week John suffered through no energy, night sweats and insomnia for a total of a 2 week detox off Methadone. Twice the time of detox off Heroin and 3 times the detox off Roxy.

This is 2015, not 1981, and PCP use is on a violent rise in our Nation’s Capital, Washington DC.

PCP (phencyclidine) is an illegal hallucinogenic drug notorious for causing violent and bizarre behaviors.

Those who are old enough will remember a huge PCP problem gripped the United States in the ‘80s. Widespread abuse caused public panic and then, somewhat mysteriously, the drug seemingly faded away.

Fast-forward to the present day; PCP usage in Washington DC, our nation’s capital, is rampant. In fact, abuse of the hallucinogen is far higher in DC than in any other major U.S. city. The disturbing trend is best seen among people who are arrested and brought in to area jails, with an overwhelming majority of DC inmates testing positive for PCP during the booking process.

PCP users rank among the smallest group of illegal drug users in the United States but this small group causes the most violent issues, according to law enforcement officers. And to add insult to injury,  there are more law enforcement officers per resident in Washington DC than anywhere else in the country. So this adds to the constant contact between users and police. Once ingested, usually by dippers, this powerful hallucinogenic causes the user to become overly aggressive and violent.

Crazy stories have surfaced like when rapper, Big Lurch, killed his girlfriend, split her chest open and ate parts of her lungs while high on PCP. He was found naked in the street covered in her blood. Yet, he recalls nothing of the incident.

This is just one of many violent and sickening stories that are fueled by this drug. Washington DC police have done their best to Crack down on open air PCP drug sales and have even made penalties extremely harsh for offenders but PCP is extremely hard to find during its export and import due to its liquid form and amber color. It resembles apple juice and is hard to detect.

  1. To be the best, you need to be trained by the best!

Let’s be honest, if you don’t care who trains you as an interventionist, it’s a crap shoot as to your success in training and skills. You should do your research and find highly skilled professionals with years of experience in the field of interventions. The best instructors are not people who read books; they are professionals who have the experience in the field that can show you the different techniques to be successful. The Addictions Academy’s instructors have many years experience in the field and have had hundreds of successful interventions using different methods and techniques.

  1. An interventionist has to be prepared before they execute.

Successful interventionists plan out their interventions. An intervention should never be rushed or spontaneous; it should be carefully planned out usually at least a week from the original call. The interventionist’s transportation, and lodging, needs to be set up, as well as the date, time, and location of the intervention.   A contract needs to be sent to the client, a deposit sent from the client, and a pre-intervention plan set up. The interventionist also needs to have a good idea of appropriate treatment facilities for the client and an escort plan to transport the client to the facility chosen. Successful interventions are well planned and thought out in advance.

  1. Don’t trust what you hear; trust what you see!

Often family members will be the initial contact with the interventionist. The family may think that they’re providing concrete facts, but more often than not, their facts are not one-hundred percent accurate. They may be enablers who have been manipulated by their addicted loved one. Perhaps the family has been lied to about the facts. The referral resource just simply may not know the facts. Even when the interventionist communicates directly with the addicted family member, they still may not get to the truth. Interventionists must trust their instincts and use all of their professional skills to make decisions on the client. Remember that body language is 55%, tones and inflections 38%, and the spoken word only 7% of trusted human communication.

  1. Interventionists don’t react on emotions.

An intervention is loaded with emotions, tears, yelling by family members, and anger. The interventionist stays professional and never reacts based on emotion. Interventionists rely on their training, experience, common sense, and wisdom. Interventions can become very heated between loved ones, prepare the family that you may ask certain people to step outside if their actions interrupt the positive flow of the interventionist’s progress. Interventionists remain professional at all times; they never get pulled into the family’s drama.

  1. Professional interventionists have to be able to understand family dynamics.

T he interventionist must be able to identify and understand the dynamics of each family unit. This information is invaluable as a tool for the interventionist’s success in handling the client and the outcome of the intervention. Most important is finding out who the enabler or enablers are in the family unit. The enabler can sabotage the whole intervention if not identified and handled. The hero in the family is another key player in the family unit. The hero will assist in the intervention with blocking the enabler. To have a successful intervention, the other players worth identifying are, the family mascot, the scapegoat, the lost child, and of course the addict or addicts. The intervention will use all this information to their advantage and even set the room up for success with a door blocker, spacers, leader, and enforcer within the family units.   1. Professional interventionists must know how to identify and handle different types of clients.

All clients are not the same! Some of the basic characteristics that interventionists screen for are genetic factors, emotional stress, psychiatric problems, and unresolved issues from the client’s past. Types of clients can include the following

  1. Plan on a smooth intervention and be ready for a rocky one.

Interventionists must be ready for the curve ball, the fastball, and the slider!  Anything can and will happen during an intervention. The interventionist must be on their toes at all times. No physical violence should be tolerated! The police may have to be called if things get out of control. They must make sure that everyone is safe. The interventionist may have to deal with denial, anger, verbal abuse, accusations, and must be prepared to follow through with consequences. The interventionist never really knows how things will go in advance. The main point is that they are ready to handle the worst possible situation and hope for smooth sailing.

  1. The interventionist must remain positive at all times.

No matter what happens, the interventionist should always remain positive and hopeful toward solution for the clients. The family has been through very hard and emotional times, they may quickly become negative. The interventionist can help set the tone and mood for the intervention by remaining positive. This should lead to a good outcome.

   9.    There are several different models of interventions.

There is a simple intervention: One person confronts addicted person Plan on a smooth intervention and be ready for a rocky one.

A Classical Intervention: educating the family, pre-intervention.

A Family System Intervention: Multiple family members addicted.

A Crisis Intervention: Stabilize the situation quickly, rehabilitation follows.

Johnson Model (Surprise Model): Often used by Interventionists.

The Arise Model: Involves the addicted person from the beginning.

The Invitational Intervention Model: Involves the whole family.

Systemic Intervention Model: Focus is on the family, highly successful with process addictions

The RAAD Intervention Model: Reading the client, assessing the situation, anticipating client needs, and directing the flow.

Case Study

1. Problem: “Jack” was a seasoned Executive in a Tech firm that was set to receive a big promotion. The problem was that Jack was a bit of a drinker. When Jack drank he claimed to black out and not remember much the next day. At the Company Party, Jack drank and not only make inappropriate sexual remarks towards a female coworker, he groped her. The Tech firm called us to discreetly come in and ‘handle the problem’ so as to avoid firing Jack (he was after all a great employee and innovator, he just had ‘this one vice’” alcohol.

Average company solution: Call the EAP provider and HR. EAP is NOT equipped to handle addiction and more importantly high profile bad behaviors that put the company in the spotlight. Plus, who actually wants to confront a high level executive on their bad behavior? TACA will.

TACA Solution: We arrive like Gladiators in Suits, have a pow wow with Jack and whisk him away to treatment or at home services where he can be afforded the tools to handle his drinking and behavioral issues. This permits the company to handle any damage control, keep a great employee and handle the bad behavior. @TACA we understand the corporate culture and can integrate ourselves discreetly, swiftly and effectively.

2. Problem: “Bert” is a First Responder. He flies the emergency helicopter to rescue individuals. Bert loves his job and needs it to feed and clothe his family of four. Bert had an accident and damaged his back. He was given opiates for pain and became addicted. Bert does not see himself as an addict, because after all these were prescribed by a doctor. To return to work, Bert must pass a urine test. He has weaned himself down to very little pain meds per day and is refusing detox, treatment or any inpatient services, because in his mind he is not ‘one of those drug addict junkies”. He also would have to take 28 days off from work, time which is not paid and his family will suffer.

Average Solution: Go to treatment or lose your job.

TACA Solution: The Doctors and staff @TACA can assist Bert in not only getting off the meds, we can assist with him staying off the meds. Using holistic psychology, vitamins and all natural products we can help him stay off the pain meds. We will teach offer yoga, acupuncture, cold laser and other healing modalities along with therapy and coaching to get his life back on track. Out of the box thinking, produces great results. We come to him and keep him out of the limelight.

3. Problem: “Janet” has a 16 year old son that is using heroin and alcohol. She has called the police twice on him when he stole her car and pawned her jewelry. She has spoken to school officials, friends and family and seems to be at wits end with what to do. She doesn’t want to send him to jail, but he is refusing treatment.

Average Solution: Send him to jail, he may “learn to stop using drugs” or send him to a 28 day adolescent program.

TACA Solution: Janet needs coaching on how to handle the addiction and education on what is going on. We offer wrap around family services to Janet to educate her, coach her and even counsel her as necessary. We will intervention on her son, send him to a specialized program for long term sobriety and behavioral health management. He can complete school there and even begin college. We will negotiate with the location for payment for Janet. We will coordinate case management services and continue working with Janet so she understands what will occur when he returns home and how to handle his behaviors. We will also coach the rest of the family and then place her son with a sober companion or a coach upon release. Addition of the family services and family education is essential in assisting her son change his behavior. By teaching the family how to change their own behavior, the client will change his.

4. Problem: “Kurt” is a CEO of a large tech firm and enjoys the use of cocaine and escorts to ‘relieve stress’. He is interested in cutting back on his use and does not want to affect his job, but he is not interested in stopping altogether. He feels his use of both drugs and sex are well deserved after 80 hour work weeks.

Average Solution: Force Kurt into treatment by threat of loss of job. Everyone will know he is in ‘rehab’.

TACA Solution: Meet the client right where they are. Contain the worst behaviors first, and ask Kurt what his goals are and consequences of serious behavioral issues in both workplace and home life. Plan to meet Kurt after work hours with our mobile rehab component and sober coach services. Accompany Kurt on any work excursions to instill healthy behavior, assist with harm reduction and ways to reduce the problematic consequences of choices. Work with him on lessening the behaviors and increasing the time between use and working on a safer manner to engage in his behaviors. When client is ready we continue to reduce the amount of drug use and use of escorts. Contain any work issues and buffer to keep company and job out of limelight.

She’s Dealing With the Death of a Loved One
If your best friend’s lost someone she loves, don’t stay away in fear that you’ll say the “wrong”
thing. What’s more important is “listening and just being there until the person processes the
loss,” explains Cali Estes, PhD, a cognitive behavioral therapist and founder of The Addictions
Coach and The Addiction Academy. “Sometimes just having coffee with them in silence is what
the person needs.”

Your Friend’s Wrestling With an Addiction
Maintaining a friendship with someone who’s has an addiction “can be exhausting,” admits Estes.
You’re gonna need to make — and keep — some strong boundaries. “Don’t give them
money or lend your car or other items,” says Estes. But DO keep offering help. You can even
research local professionals and hand your friend a list of phone numbers. “They may be irritated
with you, but stay the course,” urges Estes. “If they get help, they’ll thank you in the end.”

You’ve Had a Blowout Argument
When you and your closest friend have an argument that’s not easily resolved, “it’s important to
decide if you want to ‘be right’ and ‘win’ or if you want to get a resolution that’s workable,” says
Estes. “You generally don’t get both.” With that in mind, be realistic. “Sometimes you have to
give up control to get the desired result,” Estes notes. And that result, of course, is a friendship
that continues.

The purpose of this study is to validate the relationships between substance dependence, withdrawal, food addiction and recovery. The intention is to show how most afflicted with substance addiction have disordered eating habits that during detoxification turn into highly intense cravings for particular food ingredients. And also to illustrate how these particular attributes can be attributed to increased post-acute withdrawal, relapse, and the development of a potential food addiction.

Cali teaches ethics training because she strongly believes in an ethical practice and will only work with people who run an ethical practice. According to Cali, the industry has sharks, tuna and dolphins. Sharks are those that prey on others’ weaknesses and steal others’ clients. The tuna don’t like to work for much and expect everything to come to them, while the dolphins share their clients. For a thriving practice, swim with the dolphins and not the sharks. In the industry, there’s enough clientele for everyone without picking off other therapists’ clients.

Some therapists make the mistake of thinking that everyone is a prospective client; the truth is, not every client fits with every therapist, so therapists much assist each other in finding the right client fit, not be overprotective and conniving. It is when therapists work together that they become a community of dolphins and not sharks. Those therapists who have a niche practice tend to have more success in their practice than generalists. There is less competition in the clientele pool when you cater to a very specific demographic.

Success in practice doesn’t come from complete selflessness and sacrifice. Cali implores therapists to practice self-care, as they cannot serve others until they serve themselves first. You can serve others and be able to pay your bills at the same time. Having enough money to live on and helping others is mutually exclusive.

What people consider failures in practice are not always what they seem if you shift your paradigm of thought. If you learn from the experience, then it’s not a failure. If you’re a tuna and learn nothing, then you’ll always fail; there’s always a lesson to be learned.

Cali’s two main tenets for success are: make it happen and you can do anything. In order to do anything, you have to be the dolphin and not the tuna, and go out and get what you want. You can do anything if you persevere. The most important indicator of a successful practice is perseverance.
